Who we support

CCS works with NDIS participants of different abilities, ages, backgrounds and support needs, including people with physical or intellectual disability, autism, psychosocial disability and complex support arrangements. We look at each enquiry individually, so we can be honest with you about whether we have the right service and capacity.

Areas we serve

Face-to-face in Canberra & Queanbeyan, remote everywhere else

CCS delivers face-to-face disability support across Canberra and Queanbeyan, including Belconnen, Gungahlin, Inner North, Inner South, Woden, Weston Creek, Tuggeranong and nearby communities.

We also provide remote Support Coordination to eligible participants in Sydney, Melbourne and regional or rural Australia. Outside Canberra and Queanbeyan, CCS doesn’t offer local face-to-face support — remote coordination runs by phone, video meetings, email and other methods we agree with you. Availability depends on the service, your needs, funding and our current team capacity.

Frequently asked questions

NDIS provider questions, answered plainly

Yes. Coordinated Care Services is a registered NDIS provider based in Canberra. That’s not the same as being part of the NDIA — the NDIA manages the NDIS, while CCS is an independent provider of approved supports.

CCS provides Support Coordination, Assistance with Daily Life, Social and Community Participation, NDIS Transport, Mental Health and Autism Support, and Supported Independent Living. What’s available depends on location, funding, support requirements and current capacity.

Yes, for remote Support Coordination. Eligible participants in Sydney, Melbourne and regional or rural Australia can work with CCS by phone, video meeting, email and other agreed remote methods. We don’t provide face-to-face or direct disability supports in those areas.

Tell us what matters to you in a support worker. Where possible, CCS takes into account gender, language, cultural understanding, communication style and shared interests — alongside worker skills and availability.

Get in touch before you end an existing service. We can talk through your current arrangements, notice requirements, risks and preferred start date, and help plan a transition that protects continuity.
