Support Connection

Support Connection helps you understand the plan, connect with services and start using funded supports. It generally suits participants who need a lighter level of help to get their support arrangements established.

Coordination of Supports

Coordination of Supports offers more ongoing assistance for participants with multiple providers, changing needs, service barriers or more complex arrangements. The work can involve coordinating services, resolving points of failure, strengthening informal and mainstream supports, and building the participant’s ability to direct their own supports.

Which level applies to you

CCS currently describes these as Level 1 and Level 2 Support Coordination. Check the exact wording in your plan before support begins.

CCS provides both Support Coordination and direct disability supports. That can be convenient, but it also means choice and transparency need to be protected.

Using CCS for Support Coordination does not mean you have to use CCS for direct supports too.

Changing your support coordinator

You can choose your Support Coordination provider. If your current arrangement isn’t working, CCS can talk through your service agreement, notice period, outstanding actions, information handover and any immediate continuity risks. Don’t cancel an essential service before a safe transition is confirmed.

Local and remote Support Coordination

CCS provides local Support Coordination across Belconnen, Gungahlin, Inner North, Inner South, Woden, Weston Creek, Tuggeranong, Queanbeyan and nearby areas. Delivery may be face-to-face, phone or online, depending on participant preference, need and availability.

For participants in Sydney, Melbourne and regional or rural Australia, CCS provides Support Coordination remotely, through scheduled phone calls, video meetings, email and agreed digital tools. Support Coordination questions, answered plainly

Yes. To purchase Support Coordination with NDIS funding, the relevant support needs to be in your plan. If you’re not sure, CCS can help you find the right wording — but we can’t add funding or guarantee the NDIA will fund it.

A support coordinator can help you understand budgets and how services fit within your plan, but they don’t handle your money. Plan managers process provider invoices and handle financial administration — Support Coordination and plan management are different services.

No. We can identify options, talk through suitability and help you ask the right questions. The final choice belongs to you, or your authorised decision-maker.

Yes, within the scope of Support Coordination. We can help identify risks, contact relevant providers with consent, explore alternatives and coordinate next steps. CCS is not an emergency or crisis-response service — call Triple Zero for immediate danger, and use the appropriate health or emergency service for urgent clinical needs.

Yes, with the participant’s consent, or where a legally authorised representative is involved. We clarify who can receive information and make decisions before sharing any personal details.

Contact frequency depends on your goals, current risks, preferred communication and available funding. We agree the expected approach at onboarding, and review it when circumstances change.

Yes. You can use CCS solely for Support Coordination and choose other organisations for direct supports. Your coordinator should present options fairly and respect whatever you decide.

Yes. CCS provides remote Support Coordination for eligible participants in Sydney, Melbourne and regional or rural Australia, delivered by phone, video meeting, email and other agreed remote methods. We don’t offer face-to-face CCS Support Coordination outside Canberra and Queanbeyan.
