Support Coordination helps you understand and implement your NDIS plan. A CCS support coordinator can connect you with providers, coordinate formal and informal supports, work through barriers, keep an eye on whether services are actually working, and build your confidence to manage supports over time.

Help or supervision with personal care, preparing meals, household tasks and everyday routines at home. CCS agrees the tasks, timing, worker requirements and boundaries with you before support begins.

Best suited to: participants with relevant Core funding who need support with everyday activities at home.

Transport assistance may support travel to work, study, appointments or community activities, where it relates to the participant’s disability needs and plan. CCS confirms the purpose, arrangements and applicable charges before each trip.

Best suited to: participants with appropriate funding who cannot travel independently, or who are building travel confidence.

Mental Health and Autism Support

CCS supports autistic participants and people with psychosocial disability to build routines, manage everyday tasks, connect with community and work toward greater independence, adapted to each person’s communication, sensory, emotional and recovery needs.

Supported Independent Living

SIL is support delivered in a participant’s home for people with higher support needs. Depending on the approved plan and support arrangement, it may include personal care, help with meals and household tasks, skill development, supervision and overnight support.

Best suited to: participants with higher support needs who have SIL included in their plan, or who are exploring home and living options.
