Participants can refer themselves to CCS. Referrals can also come from family members, nominees, support coordinators, Local Area Coordinators, hospital teams, allied health professionals and other services — as long as the participant has consented, or the referrer has appropriate authority.

You don’t need to send every document in the first form. The following information helps CCS understand the request:

Referral review
CCS reviews the requested service, location, timing and initial support information.
Contact and consent
We contact the participant or authorised person using their preferred method and confirm consent to proceed.
Suitability discussion
We ask further questions about goals, funding, risks, worker requirements and current services.
Capacity check
CCS confirms whether we have the right registration, competencies and workforce capacity.
Service agreement and onboarding
If the service is a good fit, we explain fees, responsibilities, documents and the proposed start plan.
Submitting a referral doesn’t guarantee service acceptance or an immediate start. CCS will give you an honest response about suitability and capacity once we’ve reviewed the information.
Support Coordination referrals
What to include for Support Coordination
For a Support Coordination referral, include the type of Support Coordination funded, plan dates, available budget if known, immediate plan implementation needs, current providers, risks of service breakdown, and any upcoming reassessment or transition date. If the participant lives outside Canberra and Queanbeyan, note that the requested CCS service is remote only, and include preferred phone, video or email arrangements.

What to include for a SIL enquiry
For a SIL enquiry, include the participant’s preferred area, current living situation, approved home and living supports, accessibility needs, daily and overnight support requirements, compatibility considerations and target move timing. We can request relevant assessments securely once initial consent is given.
Make a SIL EnquiryThe referral form is not monitored as an emergency service. If someone is in immediate danger, call Triple Zero. For urgent clinical or mental health needs, contact the appropriate health or crisis service. Existing CCS participants with an urgent service issue should use the agreed after-hours or escalation contact.
