An NDIS plan can involve providers across different Melbourne regions, online services and wider Victorian networks. CCS provides remote Support Coordination to help eligible Melbourne participants understand their funding, get services established, and respond when support arrangements change.
CCS operates from Canberra. We don’t have a Melbourne office or a local face-to-face Support Coordination team — the Melbourne service described on this page is provided remotely.
CCS remotely supports eligible Melbourne NDIS participants to understand and implement their plans, research providers, organise service agreements, coordinate support teams, address service barriers, and prepare relevant information for NDIS planning conversations. Contact happens through phone, video, email and other agreed remote methods.
How remote coordination works

A participant’s support team may be spread across inner Melbourne, northern, western, eastern or south-eastern suburbs. Remote meetings can bring providers and authorised supporters together without anyone having to travel to one location.
When providers have waiting lists, limited catchments or can’t meet the participant’s requirements, CCS can help document the barrier, explore alternatives and coordinate interim next steps within the scope of the plan.
Remote coordination may assist with planning provider changes, discharge-related service arrangements, changes in daily support, or preparing for a different living situation. CCS cannot replace local clinical, housing, safeguarding or emergency services.

Yes, with consent or appropriate authority. Remote meetings can include relevant family members, nominees and providers, while still respecting privacy and the participant’s choices.
CCS cannot physically attend appointments in Melbourne. A local support worker, advocate, family member or other suitable service may be needed for that in-person help.
You can choose your Support Coordination provider, subject to your plan and service agreement. CCS can talk through notice, information handover, current actions and whether remote delivery is suitable, before any transition happens.
