About Coordinated Care Services

Coordinated Care Services is a registered NDIS provider based in Canberra. Since 2018, CCS has supported people with disability to exercise choice, build independence and take part in everyday community life through practical, individualised services.

Why was CCS established?

CCS was founded by Martin, who saw gaps in traditional disability service delivery and wanted to do something about them. The idea behind the organisation is simple: start with the person’s goals, preferences and lived experience, then organise support around that — not the other way round.

Our mission is to provide compassionate, high-quality and responsive support that helps people with disability, autism and psychosocial disability pursue their goals, build independence and take part in their communities.

Our vision

We want communities where people of all abilities are respected, included and able to make meaningful choices about their own lives. CCS contributes through reliable support, honest communication, advocacy, and relationships that recognise each person’s strengths.

Choice and control

Participants should understand their options and stay involved in decisions. CCS doesn’t make decisions for you just because we’re the service provider. Where CCS offers more than one relevant service, we explain the alternatives and manage any conflict of interest openly.

Safety and quality

CCS delivers services within its NDIS registration scope and quality obligations. Safe support depends on accurate information, suitable worker competencies, clear plans, supervision, incident management and open reporting of concerns. If we can’t deliver a service safely or reliably, we won’t take it on.

Working with families and support networks

Families, carers, nominees and professionals can be important parts of a participant’s support network. CCS works collaboratively with them, while respecting the participant’s consent, privacy, communication preferences and legal decision-making arrangements.

Based in Dickson, working across Canberra & Queanbeyan

From our base in Dickson, CCS supports participants across Canberra, Queanbeyan and nearby communities. Local knowledge helps us understand travel, service availability and the practical differences between regions — but every referral is still assessed on its own merits.
